丙酮、乙酰乙酸和β-羟丁酸(β-Hydroxybutyrate, β-HB)统称为酮体(ketone body),当胰岛素水平下降到非常低的水平时,发生糖尿病酮症酸中毒,关闭葡萄糖供给身体。生理反应是由脂肪酸氧化产生的乙酰辅酶A产生酮体(主要是β-羟基丁酸酯,占75%以上)。酮体生产率非常高,超过了身体利用它们作为能量来源的能力,血液浓度越来越高,称酮血症(ketonemia)。过多的酮体从尿中排出,称酮尿(Ketonuria)。因酮体中除丙酮外,均为强酸物质,过多的酮体由尿排出时,必须与体内碱储(主要是Na+)结合成盐类,因此体内碱储消耗过多,最后导致代谢性酸中毒及脱水,这种病理变化称为酮症(Ketosis)。
